Offer your perspective on the NLSA’s Inclusion Action Plan
The Newfoundland and Labrador Soccer Association (NLSA) is exploring how it can improve its policies, practices, and soccer programs. The NLSA is developing an action plan that seeks to create more respectful and welcoming environments for NLSA staff, board members, and Associations, and develop NLSA programs meet the needs of participants from different communities. External consulting group, INclusion INcorporated, were contracted to complete this project.
The NLSA is committed to creating a welcoming and inclusive soccer community and is working toward ensuring that NLSA programs and environments are fair and accessible.
This questionnaire will take approximately 10 to 12-minutes to complete. Responses will be used to help inform the development of an Inclusion Action Plan for the NLSA. All questions are optional. https://forms.office.com/r/mjd5MYKYzM
Privacy, Confidentiality, & Information Storage
Maintaining your privacy is crucial in this process. This questionnaire is being deployed and analyzed by external consultants, INclusion INcorporated and only staff and contractors of INclusion INcorporated will have access to responses. Only these external consultants will see the questionnaire responses for the purposes of analysis.
The questionnaire is hosted on Microsoft Forms as part of INclusion INcorporated’s secure file network hosted by SharePoint. It is also confidential. No personal identifying information (for example: name, birth date, address, email) will be collected, stored, or published.
Use of Information
This questionnaire will be used to inform the development of the Inclusion Action Plan for the NLSA.
Your participation in this questionnaire is completely voluntary. While you are encouraged to participate in this questionnaire you are not required to do so. You are also not required to answer all the questions if you do not feel comfortable. You can close the questionnaire at any time. The deadline to complete the questionnaire is 6:00 pm AT on June 30, 2025.
